Scenario A: “Jasmine,” a care provider with support from CDS for comorbid risk management
Background:
Jasmine is a General Practitioner who sees patients living with serious mental illness (e.g., schizophrenia, bipolar). One of her patients, Mark, is on an antipsychotic known to increase metabolic risk. Mark has a family history of type 2 diabetes, and his BMI has drifted up over the last few visits.
CDS Intervention:
Jasmine’s practice uses a CDS tool integrated into their electronic health record (EHR), which flags patients with SMI + antipsychotic medications + risk factors (family history, BMI, lab values). The system automatically prompts a metabolic health review (glucose, lipids, weight, diet/exercise), offers guidelines for interventions (dietician referral, switching to an antipsychotic with lower metabolic risk, lifestyle counselling), and tracks follow-ups.
Outcome:
Mark’s risk is recognised earlier. Jasmine works with him to adjust lifestyle; she also discusses with the psychiatrist whether an alternative antipsychotic could be considered. With quarterly reviews, Mark’s weight stabilises and his HbA1c remains within safer limits. The CDS tool also helps avoid duplicated blood tests, improves documentation, and ensures Mark’s physical health is monitored alongside his mental health.
Scenario B: “Lila,” a secondary care psychotherapist using CDS for treatment selection in depression
Background:
Lila is a psychological therapist working in secondary care. She evaluates many clients with moderate-to-severe major depressive disorder. Outcomes are variable; some benefit from psychotherapy alone, others need adjunct pharmacotherapy or more specialist intervention.
CDS Intervention:
A service using an AI-augmented CDS system (validated in the local population) that helps predict the likelihood of response to first-line therapies (e.g., CBT vs antidepressants) based on historical data from past patients: symptom profile, previous treatment history, comorbid anxiety, sleep disturbance, age, etc. It also provides guidelines for combination therapy, warns of interactions, and helps suggest next steps if no improvement after 4-6 weeks (stepped care).
Outcome:
Lila uses the CDS predictions as one input in her case formulations. She discusses the options with her clients more clearly: “Based on past clients like you, these treatments had better outcomes in these conditions.” For some clients, this leads to starting combined therapy earlier; for others, sticking to psychotherapy but with monitoring. Treatment response is more efficient; fewer clients spend weeks with ineffective therapy, and drop-outs are reduced because expectations are clearer.

- Define the problem before buying the tool
What specific decision(s) are hard right now? Where are gaps in care? What outcomes do you want to improve? This helps you select or design CDS features that are relevant. (e.g., screening, risk, treatment selection, physical health comorbidity.) - Involve users early and continuously
Clinicians, therapists, nurses, psychiatrists, and service users, everyone’s perspectives matter. Co-design workshops, feedback loops help ensure the tool matches real workflow and culture. Research shows this increases long-term acceptance (Newton et al., 2023). - Ensure integration & usability
Integrate with your EHR or existing digital systems. Minimise extra clicks. Make alerts non-interruptive unless they are safety-critical. Be mindful of presentation: clear, simple, actionable guidance. - Transparency, explainability, and trust
Let users see the rationale, evidence sources, and limitations of the CDS. Allow overrides. Build regular reviews and updates into governance. - Monitor, evaluate, iterate

However, with these advances comes greater responsibility to build tools that are safe, fair, ethical, and enhance human care rather than automate away the relational aspect that is central to mental health.
